At SABI Concierge Physical Therapy ("SABI," "we," "our," or "us"), we know that privacy — including the privacy of your health information — matters. This policy covers what we collect through our website and in the course of your care, how that information gets used and shared, and the choices available to you. By using our website or scheduling services with us, you're acknowledging that you've read and understood this policy.

2. Protected Health Information

As part of providing physical therapy care, we create and keep records containing your protected health information ("PHI") — things like your intake history, evaluation findings, and treatment notes. This information is collected and stored through Jane, our HIPAA-compliant electronic medical records (EMR) and practice management platform. When you book your first visit, Jane's intake process will ask you to review and sign our HIPAA Notice of Privacy Practices, which governs the use and disclosure of your PHI in more detail than this website policy. This information is handled in line with applicable health privacy laws, and we don't use or disclose it beyond what's described below without your authorization.

SABI is a private-pay practice — we do not bill insurance, so your health information is not submitted to or shared with insurance carriers for claims processing.

3. How We Use Your Health Information

We may use or share your health information to:

• Coordinate and provide your treatment, including communicating with a referring physician or another provider involved in your care.
• Support the everyday operation of our practice — quality reviews and staff training among them.
• Send appointment confirmations and scheduling reminders, including by text message where you've opted in. We do not share text opt-in or consent data with outside parties for marketing purposes.
• Comply with a subpoena, court order, workers' compensation requirement, or other legal obligation.
• Respond to an emergency — in that case, we'll seek your authorization as soon as reasonably possible afterward.

4. Sharing With People Involved in Your Care

Unless you tell us otherwise, we may share relevant portions of your health information with a family member, partner, or friend who is helping coordinate or pay for your care. If you're not able to state a preference at the time, we'll rely on our professional judgment to decide what, if anything, is appropriate to share and in your best interest.

5. Website & Personal Information

Our website, www.sabinyc.com, does not have a contact or booking form, and we do not collect your name, email, or phone number simply by your visiting the site. If you email, call, or text us using the links on our site, you're sharing that contact information with us directly, and we use it only to respond to you.

Appointment booking and patient intake are handled through Jane, our EMR and practice management platform, at a separate site (sabinyc.janeapp.com). When you book a visit, Jane collects the information needed to schedule and treat you — including your name, contact details, and health information — as described in Section 2 above. On www.sabinyc.com itself, we collect only general usage data — such as browser type, device, and pages visited — through Google Analytics and Vercel Speed Insights, using cookies and similar tools. This helps us understand how visitors use our site and how it performs. We do not sell your information to third parties.

Google Analytics collects usage data using cookies and similar identifiers (such as mobile advertising identifiers), and this data may be shared with Google. You can learn more about how Google collects and processes data at How Google uses information from sites or apps that use our services, and you can opt out of Google Analytics tracking using the Google Analytics Opt-out Browser Add-on. By continuing to use our site, you consent to this use of cookies and analytics.

6. Your Rights

You have the right to:

• Request a copy of your treatment and billing records (a reasonable fee may apply to cover copying costs).
• Ask us to limit how a specific piece of your health information is used or shared — we'll consider every request, though we're not always able to agree to a restriction.
• Withdraw consent for text or email communications at any time.
• Raise a concern directly with us, or with the U.S. Department of Health and Human Services, if you believe your privacy rights have been violated.

7. Security

We maintain reasonable administrative, physical, and technical safeguards designed to protect your information from unauthorized access, use, or disclosure.

8. Third-Party Links

Our site links out to Jane, our online booking and EMR platform, and to your own email or phone app when you use our contact links. Once you leave our site, that third party's own privacy practices take over — we'd encourage you to give them a look.

9. Updates to This Policy

We may revise this policy from time to time to reflect changes in our practices or the law. The version posted here is always the current one, and continuing to use our website or services after an update means you accept the revised terms.

Notice Regarding Electronic Communication: Please be aware that standard email is not a secure form of communication and carries some risk of interception by unauthorized third parties.
